ABSTRACT

ObjectiveTo establish a multi index fusion hand grip fatigue prediction model to evaluate the power-assisted effect of the glove exoskeleton prototype for extravehicular clothing. MethodsBP neural network algorithm was used to establish a hand fatigue prediction model. The related factors of hand fatigue were determined with isometric grasping fatigue experiment, and the input variables of BP neural network were determined as cylinder diameter, grasping force, grasping duration and root mean square of electromyography. The fatigue data corresponding to variables of each group were obtained through experiments and subjective fatigue measurement scales, and a fatigue evaluation model based on multi-source fusion of BP neural network algorithm was established. The relationship model between fatigue and assistance effect was established, and the assistance effect of the exoskeleton prototype was evaluated through the degree of fatigue relief. ResultsThe correlation coefficient was 0.974 between the predicted results of the model and the target value. Moreover, it effectively predicted the assistance effect of different prototypes. ConclusionThe BP neural network model established by combining the grasping strength, grasping object parameters and human electromyography can predict hand fatigue, which can be used to evaluate the assistance effect of glove exoskeleton and other hand aids.

ABSTRACT

Objective:The purpose of the study is to investigate how to select lowest instrumented vertebra (LIV) in posterior spinal corrective surgery for Lenke 1A/2A adolescent idiopathic scoliosis (AIS) patients,and to further identify the risk factor for postoperative distal adding-on.Methods:FromJanuary 2008 to January 2014, a total of 85 Lenke 1A/2A AIS patients with one level proximal to last substantially touching vertebra (LSTV-1) selected as LIV were enrolled in the study. There were 45 Lenke 1A and 40 Lenke 2A, and 70 females and 15 males. The average age of surgery was 14.4±2.2 years (10-18 years). They were followed up over 2 years. The upright posteroanterior and lateral radiographs were performed preoperatively, immediately after surgery and at the final follow-up. Several radiographic parameters were measured such as Cobb angle, thoracic curve length, apex location, LIV rotation, deviated distance of LIV from central sacral vertical line, coronal balance and sagittal balance, etc. Distal adding-on was defined as a progressive increase in the number of vertebrae in the distal curve at the last follow-up. Patients were classified into adding-on and non adding-on group. The risk factors associated with the incidence of adding-on were analyzed. Subgroup analysis were further performed according to the curve type.Results:The mean duration of follow-up was 37.8±16.3 months (24-95 months). The average Cobb angle of main thoracic curve was 51.9°±6.8° (42°-85°). At the last follow-up, 36 patients (42.4%) had ideal outcome without distal adding-on. For Lenke 1A patients, the risk factor for adding-on included: low Risser ( t=2.730, P=0.005), long thoracic curve ( t=1.930, P=0.030) with low apex ( t=1.734, P=0.045), preoperative large rotation and deviation of the LSTV-1 ( t=2.319, P=0.013; t=3.288, P=0.001), and preoperative coronal imbalance ( t=1.729, P=0.046). For Lenke 2A patients, the risk factor for adding-on included: low Risser ( t=2.246, P=0.015), preoperative large rotation and deviation of the LSTV-1 ( t=2.534, P=0.008; t=1.972, P=0.028), and preoperative coronal imbalance ( t=1.702, P=0.048). Conclusion:When choosing LSTV-1 as LIV, skeletal immaturity, large rotation and deviation of LSTV-1 and preoperative coronal imbalance are risk factors for distal adding-on in Lenke 1A/2A curves; Also, long thoracic curve with low apex is associated with distal adding-on in Lenke 1A curves. Therefore, for skeletal immature patients with large rotation and deviation of LSTV-1, preoperative coronal imbalance and long thoracic curve with low apex,the 'LSTV’ rule should be followed to decrease the incidence of distal adding-on. While in other case, it could safely distally stop at LSTV-1.

ABSTRACT

Objective:To investigate the radiological presentations in relation to different ages in scoliosis associated with Chiari malformation typeⅠ(CMI).Methods:A retrospective analysis was performed on 80 patients diagnosed with scoliosis associated with CMI from June 2010 to June 2018, who were classified into three groups according to their age: Children(age≤10 years), Adolescents (age 11-18 years) and Adults (age≥19 years). Curves were classified into typical and atypical patterns in the coronal plane. The coronal and sagittal radiographical parameters were measured in the three groups. Moreover, cerebellar tonsillar descent and syringomyelia patterns were measured on MRI, and the parameters among the three groups were compared statistically.Results:The incidence of atypical curve patterns in Children (10 patients), Adolescents (44 patients) and Adults (26 patients) was 30.0%, 15.9%, and 50.0%, respectively (χ 2=2.654, P=0.265). There was no statistical difference in the distribution of curve patterns among CMI patients with different age. In the coronal profile, Cobb angle ( F=16.751, P<0.001) and flexibility ( F=3.285, P=0.044) of main curve, Cobb angle of secondary curve ( F=9.805, P<0.001) and coronal balance(CB) ( F=5.249, P=0.007) showed statistical difference. The elderly patients tended to have larger Cobb angle of main and secondary curve with worse flexibility of main curve, and CB in Adolescents was better than the other two groups. In the sagittal profile, TK ( F=4.324, P=0.017), LL ( F=4.590, P=0.013), PI ( F=5.501, P=0.006), and PT ( F=3.220, P=0.045) showed statistical difference in the three groups, which were increasing significantly with aging. MRI parameters showed that younger patients were more likely to have a higher degree of cerebellar tonsillar descent (χ 2=18.479, P<0.001) and distended syringomyelia (χ 2=23.074, P=0.003). Conclusion:With aging, Cobb angle of main curve is progressive, and the flexibility is worse, suggesting that early surgical intervention should be performed to reduce the risk of surgery. In addition, cerebellar tonsillar descent and syringomyelia in elderly patients are milder than young patients, indicating that there might be spontaneous remission.

ABSTRACT

Objective:To evaluate the efficacy and safety of water injection therapy combined with medical moisturizing repair cream in the treatment of rosacea.Methods:Total of 100 patients with types I and II rosacea in the Department of Dermatology, the First Affiliated Hospital of Army Medical University were randomly divided into three groups: comprehensive group (treatment group), hydrate group (control group 1) and moisturizing group (control group 2). Comprehensive group and water light group were given water injection therapy, once a month, a total of 3 times of treatment; in the comprehensive group and the moisturizing group, topical medical moisturizing repair cream was used once in the morning and in the evening, for a total of 12 weeks. After unified training, the therapists evaluated the skin lesions according to the scoring criteria and recorded the adverse reactions, for a total of 12 weeks.Results:After 12 weeks of treatment, the integral difference values before and after treatment were 8.01± 2.43, 7.68±2.16 and 5.63 ±2.19 respectively in the comprehensive group, the water injection therapy group and the moisturizing group, with statistically significant differences ( P<0.05). The effective rates of the three groups were 94.38%, 89.91% and 48.12%. No serious adverse reactions were observed. Conclusions:The water injection therapy combined with medical moisturizing and repairing cream is safe and effective in the treatment of rosacea; it is more effective than the moisturizing and repairing cream.The water injection therapy combined with medical moisturizing and repairing cream has advantages but is not obvious enough.

ABSTRACT

We utilized a multi-step derivatization gas chromatography-mass spectrometry method for the determination of common amino acid enantiomers, combined with deuterated hydrochloric acid hydrolysis, to identify nine trace D-amino acids in thymalfasin. We optimized the conditions for multi-step derivatization, the volume of reagent for redissolving samples, and the conditions for chromatography and mass spectrometry with isopropanol and trifluoroacetic anhydride as derivatization reagents, and validated the procedure, including sensitivity, linear range, precision, accuracy and recovery. Sixteen pairs of D/L-amino acids and glycine derivatives were separated within 29 min, with the limit of quantification as low as 0.09-2.79 μmol·L-1. Nine amino acid derivatives of thymalfasin showed a good linear relationship within the concentration range examined (r2>0.992 3). The precision results showed that RSD values were less than 10.90%. Accuracy test results of a reference substance ranged from 76.69% to 128.18%. Average recoveries of spiked samples ranged from 70.41% to 125.39%. For the nine D-amino acids assayed, D-Asp and D-Glu content in six batches of thymalfasin were highest, ranging 0.41%-0.49% and 0.25%-0.33%, respectively, with others less than 0.25%. The method is sensitive, efficient and reliable, available for seventeen common amino acids and their enantiomers, and works well with simultaneous determination of nine trace D-amino acids in thymalfasin, providing a reference for the comprehensive control of racemic peptide impurities in this synthetic polypeptide drug.

ABSTRACT

Objective To analyze the clinicopathological features and prognostic factors of patients with breast invasive ductal carcinoma with extensive intraductal components (EIC).Methods The clinical data of 182 patients of early breast invasive ductal carcinoma undergoing breast-conserving surgery were analyzed retrospectively in the Second Affiliated Hospital of Soochow University,and the clinicopathological features and prognosis of two groups of breast cancer patients with EIC positive (n =23) and EIC negative (n =159) were compared.Results In the EIC positive group,ipsilateral breast tumor recurrence (IBTR) was 21.7% (5/23) and the distant metastasis rate was 4.3% (1/23);in the EIC negative group,ipsilateral breast tumor recurrence (IBTR) was 1.9% (3/159) and the distant metastasis rate was 3.8% (6/159).The Kaplan-Meier curve showed that the 5-year local recurrence-free survival rates in the EIC positive group was lower than that of the EIC negative group (78% vs.97.7%) (P < 0.001),and the difference was statistically significant,but the 5-year distant metastasis-free survival rates in the two groups was similar (95.7% vs.96.2%) (P =0.892).COX multivariate analysis showed that EIC was an independent risk factor for local recurrence after breast-conserving surgery in breast invasive ductal carcinoma patients.Conclusions EIC increases the local recurrence rate in patients with breast invasive ductal carcinoma undergoing breast-conserving surgery.

ABSTRACT

Stress urinary incontinence (SUI) is a common disease which influences the middle-aged and elderly women. The damage SUI brings to patient's body and the pressure to their mind have gradually attracted attention. Early stage SUI can be cured with medicine and rehabilitation, but patients with terminal stage SUI need to undergo surgical operation. Ultrasound, urodynamic examination and pad test are commonly used in diagnosing SUI. The current research status and progresses in ultrasonic and clinical diagnosis of SUI were reviewed in this article.

ABSTRACT

Objective To explore the difference in curative effect,safety of operation and quality of life between patients with esophageal carcinoma treated by the conventional open surgical approach and those treated by laparoscopic radical resection.Methods A total of 185 patients underwent esophageal cancer surgery in our hospital from January 2010 to December 2013 were retrospectively analysed,including 90 cases accepted laparoscopic radical resection (the minimally invasive group) and 95 patients accepted conventional open radical resection (the traditional group).The perioperative indexes,postoperative survival rate,complications and quality of life between the two groups were recorded and compared.Results No statistically significant difference was found in the operation time and number of lymph node dissected in patients between the two groups (P>0.05).Compared with the traditional group,the amount of surgical bleeding,percentage of patients using postoperative ventilator,length of postoperative hospital stay and pain score in the minimally invasive group were significantly lower,while the postoperative lung function was better,there were statistically significant differences (P<0.05).The postoperative complication rate in the minimally invasive group (13.33%) was lower than that in the traditional group (25.26%),there was statistically significant difference (P<0.05).Three months after operation,the scores of dimensions,including physical function,role function,emotional state,cognitive function and social function,in the minimally invasive group were higher than those in the traditional group (P<0.05);the scores of dimensions,including fatigue state,nausea and vomiting symptoms,pain,insomnia,difficulty breathing,constipation,diarrhea and loss of appetite,in the minimally invasive group were lower than those in the traditional group (P<0.05).The 1-year,2-year and 3-year survival rates after operation in the minimally invasive group were 88.89%,73.33% and 63.33% respectively,which in the traditional group were 80.00%,65.26 % and 55.79 % respectively,there was no statistically significant difference between the two groups (P>0.05).Conclusion The effect of laparoscopic radical resection of esophageal cancer is reliable,which has advantages of less operative trauma,quicker recovery,fewer complications,high quality of life after operation.

ABSTRACT

Objective To evaluate the efficacy of combined therapies of ablative fractional Er ∶YAG laser (2,940 nm) and ALA-PDT on refractory verruca plana.Methods 120 cases of refractory verruca plana patients were randomly divided into two groups:60 cases in the control group and 60 cases in the experimental group.The control group used 10% ALA-PDT with LED irradiation of a power density 70 of mW/cm2 at a distance of 20 cm,which lasted for 20 min each time.The experimental group was treated with ablative fractional Er ∶ YAG laser (2,940 nm) first with the fluence of 500P/cm2 and short pulse duration,and then treated 10% ALA-PDT as mentioned before.Every patient was treated twice at two-week intervals.Three independent investigators evaluated subject outcomes at 3 months post-treatment including efficacy and side-effects.Results The effective rate of the experimental group was 86.44 % at 3 months post-treatment after one to two times.Meanwhile,the control group was 59.65 %.The clinical outcome of experimental group was better than the control group.There was significant difference between the two groups (P<0.05).The recurrent rate of experimental group was 3.39% and 12.30% in control group.There was significant difference between the two groups (P<0.05).There were no obvious side-effects in both groups.Conclusions Ablative fractional laser with low fluence promotes the transdermal absorption of ALA and enhances the efficacy of PDT.

Background Cataract with preexisted dry eye is a common eye disease.It is speculated that preservative-free artificial tears can inhibit inflammatory procedure,prevent postoperative eye infections and reduce dry eye symptoms.However,relevant literature is rare up to now.Objective This study was to evaluate and compare the therapeutic effects between preservative-free or preservative sodium hyaluronate combined with fluorometholone eyedrops for cataract with dry-eye.Methods A randomized controlled single-blind clinical study was performed.Sixty patients with dry-eye syndrome who was going to receive surgery for cataract removal were enrolled in Yuhuangding Hospital from January to December 2015 under the informed consent.The patients were randomly divided into the test group and control group.Preservative-free or preservative 0.1% sodium hyaluronate combined with 0.1% fluorometholone eye drops were topically administered in the eyes of the test group and control group,respectively.Ocular Surface Disease Index (OSDI) score,breakup time of tearfilm (BUT),Schirmer Ⅰ test (S I t),corneal fluorescein staining,impression cytology,goblet cell density and the concentrations of interleukin (IL)-1β,tumor necrosis factor (TNF)-α,catalase (CAT),superoxide dismutase 2 (SOD2) in tears were evaluated and compared.Results There were significant differences in gender,ages,OSDI scores,BUT,S I t value,corneal fluorescein staining scores,impression cytology findings,and goblet cell density between the two groups (all at P>0.05).OSDI,corneal fluorescein staining scores and imprint cellular level were evidently reduced,and BUT,S I t values and goblet cell density were significantly increased 1 month and 2 months after operation in comparison with the baseline values in the test group (F =13.058,8.027,3.755,21.652,70.962,92.354,all at P < 0.05).The concentrations of IL-1β and TNF-α in tears of the test group were significantly lower,and CAT and SOD2 in the tears of the test group were significantly higher than those in the control group 1 month and 2 months after operation (F=18.731,9.070,15.357,351.359,all at P>0.05).Conclusions 0.1% preservative-free sodium hyaluronate combined with 0.1% fluorometholone eyedrops can relieve the symptoms and signs of dry-eyes following cataract surgery by playing antiinflammatory and antioxidant effects.

ABSTRACT

In order to obtain the expression of ginsenoside biosynthetic pathway related enzyme gene in ginseng hairy root under the control of elicitors, methyl jasmonate (MeJA) was added exogenously as elicitors. Ginseng hairy root clones induced by 4-year-old ginseng root was used as material, total saponin content in ginseng hairy root before and after MeJA treatment was determined by vanillin-sulfuric acid colorimetry, Meanwhile, relative expression of squalene synthase genes, squalene epoxidase genes, oxidized squalene cyclase genes, dammarenediol synthase genes, β-amyrin synthase genes, cycloartenol synthase genes before and after MeJA treatment were determined by Real-time PCR. The optimum conditions of MeJA which added to ginseng hairy root were obtained, the optimum additional concentration was 6×10⁻⁴ μmol•L⁻¹, the optimum additional time was 22 d, and the optimum action time was 5 d. The addition of MeJA could improve the enzymatic activity of peroxidase (PPD), catalase (CAT) and peroxidase (PPD) in ginseng hairy root. The expression of SQS,SQE,OSC,DS and β-AS genes of ginsenoside biosynthetic pathway increased significantly after MeJA treatment, while the change of CAS gene expression were not significant. The expression of key enzyme SQS,SQE,OSC,DS and β-AS genes in ginsenoside biosynthetic pathway was consistent with the changes of PPD,CAT,PPO enzymatic activity.

Fifteen tissues of 4-year-old fruit repining stage Jilin ginseng were chosen as materials, six kinds of monomer saponins (ginsenosides Rg1, Re, Rb1, Rc, Rb2 and Rd) content in 15 tissues was measured by HPLC and vanillin-sulfuric acid method. The relative expression of FPS, SQS, SQE, OSC, β-AS and P450 genes in 15 tissues was analyzed by real-time PCR. The correlations between ginseng saponin content in 15 tissues of Jilin ginseng and biosynthetic pathway -related genes were obtained. The results showed that was a synergistic increase and decrease trend of positive linear correlation among six kinds of monomer saponin content, and there was a significantly (P < 0.01) positive correlation between monomer saponin content and total saponins content. Monomer saponin content and 6 kinds of enzyme gene correlation were different. Biosynthesis of ginseng total saponins and monomer saponin were regulated by six kinds of participation ginsenoside biosynthesis enzyme genes, the expression of these six kinds of genes in different tissues of ginseng showed collaborative increase and decrease trend, and regulated biosynthesis of ginseng ginsenoside by group coordinative manner.

ABSTRACT

Objective To analyze the effect of dexmedetomidine hydrochloride injection on patients with craniocerebral disease who has no artificial airway in the process of bronchoalveolar lavage treatment.Methods Forty-six patients (age 17-28,average age 56.6±9.2,26 men and 20 women) with craniocerebral disease who has no artificial airway were selected,and were treated by bronchoalveolar lavage for lung infection.The patients were randomly divided into two groups,control and test group.The control group (n=23) received midazolam for sedative and the test group (n=23) received dexmedetomidine hydrochloride for sedative while they were in the process of bronchoalveolar lavage treatment.Heart rate,mean arterial pressure and blood oxygen saturation of fingers collected from patients before and during the process of bronchoalveolar lavage were compared.Results In the process of bronchoalveolar lavage treatment,the minimum blood oxygen saturation of finger artery from the control group was lower than that from the test group,the fastest heart rate from the control group was greater than that from the test group,and the lowest mean arterial pressure from the control group was lower than that from the test group (P<0.05).In two groups,heart rate in the process of bronchoalveolar lavage treatment was faster than that from before the treatment,while both mean arterial pressure and blood oxygen saturation of finger artery were decreased (P<0.05).Conclusions Continuous intravenous pumping of dexmedetomidine hydrochloride on patients with craniocerebral disease who has no artificial airway during the process of bronchoalveolar lavage treatment is effective and safe,and it has less inhibitory effect on respiratory function and blood pressure.

ABSTRACT

<p><b>BACKGROUND</b>Macrophage-inducible C-type lectin (MINCLE) is an important member of C-type lectin superfamily, which has been shown evidence for susceptibility to arthritis in animal models. We aimed to investigate the possible association of MINCLE with rheumatoid arthritis (RA) susceptibility in Chinese Han population.</p><p><b>METHODS</b>Haplotypes from HapMap database (Chinese Han Beijing, CHB) were used to select tag-single nucleotide polymorphism (SNP) (r(2) = 0.8) residing in MINCLE gene. A total of 563 patients with RA and 404 healthy controls were TagMan genotyped for SNP rs10841845. Association analyses were performed on the whole data set and on RA subsets based on gender difference and the status of anti-cyclic citrullinated peptide (anti-CCP) antibody in RA patients. Association statistics were calculated by age and sex adjusted logistic regression.</p><p><b>RESULTS</b>Overall, MINCLE SNP rs10841845 was not associated with susceptibility to RA. However, following anti-CCP stratification, rs10841845 GG genotypes conferred a significantly protective effects against anti-CCP-positive RA (OR 0.65, 95%CI 0.430 - 0.995, P = 0.048). Following gender stratification, SNP rs10841845 G allele appeared to insert its RA protective effect only in male patients, both at allele level (G vs. A OR 0.66, 95%CI 0.46 - 0.93, P = 0.018) and at genotype level (GG vs. AA+AG, OR 0.429, 95%CI 0.20 - 0.95, P = 0.036). Notably, the male RA protective effect of rs10841845 G allele was only seen in anti-CCP-positive RA (G vs. A: OR 0.64, 95%CI 0.43 - 0.96, P = 0.029; GG vs. AA+AG: OR 0.375, 95%CI 0.14 - 0.94, P = 0.038). Furthermore, we observed a significant reduction of Disease Activity Score (DAS) 28 score (3.91 ± 0.70 vs. 5.66 ± 0.31, P = 0.022) and serum C-reactive protein levels (31.64 ± 24.13 vs. 91.80 ± 12.02, P = 0.012) in male anti-CCP-positive RA patients carrying rs10841845 GG genotype, compared with patients carrying AA+AG genotypes.</p><p><b>CONCLUSIONS</b>Our study provides the evidence for a gender specific association between MINCLE rs10841845 and RA susceptibility. The SNP rs10841845 G allele appears to have protective effect against anti-CCP-positive RA and confer reduced RA activity in men.</p>

ABSTRACT

Objective To evaluate the efficacy and safety of 308-nm excimer light in the treatment of stable vitiligo. Methods Thirty patients with stable vitiligo were enrolled in this clinical trial. All the subjects received the treatment with 308-nm excimer light on a twice-weekly schedule for 3 months. Results The repigmentation rate was 95.0%, 75.0% and 66.7% for lesions in the face and neck, trunk and limbs, with the treatment sessions averaging 10.22 ± 1.60, 19.10 ± 2.38, 37.74 ± 3.06, respectively, and accumulative irradiation dose averaging 7.50 ± 3.45, 10.60 ± 1.01, 18.56 ± 3.05 J/cm2 respectively. Significant differences were observed in the repigmentation rate and treatment sessions between the lesions in the face and neck, trunk and limbs (all P < 0.05). No severe side effects were seen during the treatment. Conclusion 308-nm excimer light is effective and safe for the treatment of vitiligo.

ABSTRACT

Objective To compare the efficacy and safety of excimer laser 308 nm phototherapy alone and the combination of excimer laser 308 nm and topical application of vitamine D3 alanogue tacalcitol in the treatment of vitiligo. Methods Seventy-eight patients with vitiligo were enrolled in the single-blind clinical trial, treated with excimer laser 308 nm. The lesions were devided into two groups: patients in the experimental group were instructed to use tacalcitol ointment and the control group were applied with placebo ointment. The lesions were evaluated once per month and photos taken for analyses of clinical effects. Results The results in different locations were compared, the effective rates of the experimental group in cephalofacial site, trunk and limbs were 93.51%, 84.16 % and 42.35 %, respectively. The effective rates of control group in opposite and adjacent sites were 90.9 %, 77.45 % and 34.15 %, respectively (P < 0.05). The comparison of results in different types of lesions indicated that the effective rate of the experimental group in vitiligo vulgaris and segmental vitiligo were 73.81% and 84.00 %, respectively. The effective rate of control group in vulgaris and segmental vitiligo were 86.8 %, 77.45 % and 34.15 %, respectively (P <0.05 ). The comparison of results in radiation times and doses of phototherapy showed that the radiation time and dose on the time of initial pigment regeneration were (16. 15 ± 3.22)times and (4.40 ± 5.03)J/cm2 in the experimental group, while ( 18.56 ± 3.50) times and ( 6.60 ± 1.01 ) J/cm2 ( P < 0.05 ) in the control group, the time and dose on the time of apparent effect were ( 20. 36 ± 1.50 ) times and ( 7.50 ± 3.54 ) J/cm2 in the experimental group, and (21.68 ± 2.40) times and( 8.80 ± 9.24)J/cm2 (P < 0.05 ) in the control group. Conclusion Application of tacalcitol ointment in combination with twice-weekly 308 nm excimer laser light phototherapy is an effective alternative treatment for patients with generalized vitiligo.

ABSTRACT

<p><b>BACKGROUND</b>Antibodies against type 3 muscarinic acetylcholine receptor (M3R) are involved in the pathogenesis of Sjögren's syndrome (SS), but the clinical value of them in SS patients has been controversial. The aims of this study were to: (1) establish an improved enzyme-linked immunosorbent assay (ELISA) to detect IgA antibodies against M3R; (2) evaluate the value of IgA antibodies against the second extracellular loop of M3R205-220 (c2M3RP) in diagnosis of SS.</p><p><b>METHODS</b>To increase the ELISA sensitivity, c2M3RP was coupled to bovine serum albumin (BSA) by the glutaraldehyde method and a 96-well microplate was treated by ultraviolet rays before coated. Concentrations of anti-c2M3RP, anti-SSA, and anti-SSB were measured in the sera of 240 individuals: 91 patients with primary SS and 149 controls (16 secondary SS, 27 systemic lupus erythematosus, 40 rheumatoid arthritis and 66 healthy controls). Diagnostic properties of anti-c2M3RP were determined by receiver-operating characteristic curve analysis.</p><p><b>RESULTS</b>The prevalence of serum IgA anti-c2M3RP antibodies in patients with pSS (46%, 42/91) was significantly higher than that in patients with systemic lupus erythematosus (19%, 5/27), in rheumatoid arthritis (15%, 6/40) and in healthy controls (5%, 3/66). However, there was no significant difference between the two SS groups (P = 0.727). The diagnostic performance of IgA anti-M3RP antibodies was similar to anti-SSA assay, but had 22% higher sensitivity than anti-SSB. By analyzing of IgA anti-c2M3RP antibodies, combination of anti-SSA and anti-SSB resulted in increased sensitivity, whereas their specificity was not significantly changed.</p><p><b>CONCLUSIONS</b>The improved anti-c2M3RP ELISA is a novel, sensitive, and specific serological test for the diagnosis of SS. The combined application of anti-c2M3RP, anti-SSA and anti-SSB tests can improve the laboratory diagnosis of SS. The IgA anti-c2M3RP antibodies may serve as a novel diagnostic marker for SS.</p>

ABSTRACT

<p><b>BACKGROUND</b>To determine the relationship between human papillomavirus infection, cervical carcinoma, pre-cancerous lesion and condyloma acuminatum.</p><p><b>METHODS</b>From January 2004 to August 2005, 1086 inpatients in department of dermatology and department of gynaecology and obstetrics in Southwest Hospital and No. 302 Hospital with cervical lesions and condyloma were reviewed. All specimens were detected for HPV-DNA using techniques of Gene Array and fluorogenic quantitative polymerase chain reaction (FQ-PCR). All detections of HPV-DNA were performed in the first admission before the patients underwent any examination or treatment.</p><p><b>RESULTS</b>The positive rates of HPV-DNA detection were 100% in cervical intraepithelial neoplasia (CIN) I, and II and cervical carcinoma. Among these, the main subtype was HPV 16. But some of the patients were found to be positive for more than 2 subtypes of HPV. While the commonest HPV subtype was HPV 18 in endometrial cancer. Some of the patients were detected to be positive for more than 2 subtypes of HPV. In 636 female patient with condyloma acuminatum, the infection rates of HPV6, HPV11 accounted for 44.97% and 29.40%, respectively, HPV 16 and/or HPV 18 infection constituted a small percentage. In a few cases, infection with more than 2 subtypes was detected.</p><p><b>CONCLUSION</b>Cervical carcinoma including pre-cancerous lesion differs from condyloma acuminatum in dominate infectious subtype of HPV. The former is mainly associated with HPV 16 and HPV 18 infections, and the latter mainly associated with HPV 6 and HPV 11 infections. But in both of the above lesions, a mixed infection with more than 2 types may occur and make the pathological changes and clinical treatment more complicated. The early diagnosis and supervision of HPV infection may be of great value for improvement of prognosis and quality of life.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study the effect of Shenmai Injection (SMI) on the clinical efficacy of noninvasive positive-pressure ventilation (NPPV) in treating patients with severe respiratory failure (RF) caused by chronic obstructive pulmonary disease (COPD).</p><p><b>METHODS</b>Patients in the control group (n=23) were treated with conventional therapy plus NPPV, and those in the treatment group (n=25) received the same treatment but with intravenous infusion of SMI 30 ml once a day additionally. The treatment lasted for 10 days. Arterial oxygen saturation (SaO2) was continuously monitored in all patients, arterial blood gas values was determined before treatment and at the 6th hour, the 24th hour, the 3rd day and the 7th day after treatment, and consciousness, breath, heart rate and severity of dyspnea were observed during the treating period.</p><p><b>RESULTS</b>All patients in the treatment group were markedly improved and discharged from hospital, while in the control group 20 were improved and discharged from hospital, but the other 3 failed to be remitted and received the conventional invasive mechanical ventilation. The duration of hospitalization and ventilation were 11.3 +/- 2.5 d and 5.8 +/- 2.8 d in the treatment group, and 13.2 +/- 2.7 d and 7.5 +/- 3.0 d in the control group, showing significant difference in the two indexes between the two groups (P < 0.05). The dyspnea score and the arterial blood gas values determined on the 3rd and 7th day were significantly improved in the treatment group than those in the control group (P < 0.05 or P < 0.01). The incidence of endotracheal intubation caused adverse reaction, such as facial-skin crush wound, abdominal distention and contaminated infection in the treatment group was obviously lessened as compared with the control group.</p><p><b>CONCLUSION</b>SMI could shorten the duration of hospitalization and mechanical ventilation, reduce trachea cannulation, and decrease the incidence of complications and adverse reactions in the patients with severe RF due to COPD treated with NPPV.</p>
